1. Mail: Complete the order form and return it, with payment, to CNYHSA.

2. Online via PayPal: (All online orders are subject to a shipping and handling fee)

All proceeds from the sale of these bracelets will benefit the CNY HIV Care Network and its HIV/AIDS Quality of Life Programming which includes programs that are not elgible for federal or state funding. The CNY HIV Care Network is a program of Central New York Health Systems Agency (CNYHSA).
